Students who complete their internship at the Health Department of Northwest Michigan (HDNW) will work in our Child and Adolescent Health Program, providing individual and group therapy to students within a school system, ages K-12. Students seeking an internship for 1 or 2 semesters must complete this Student Observation/Internship Request Form and provide the following documents:
- Cover Letter
- Resume
- Course syllabus, if applicable
- Specified course requirements, if applicable
